BASIC FUNCTION: The Community Health Worker (CHW) will be responsible specifically to mitigate SNAP and Medicaid benefit applications due to loss based on HR1 Statutory program changes in work requirements. Will be able to serve as on-site engagement with our Food Pantry clients, Mobile Farmer s Market Truck, and Farmer s Market and assisting with overall Case Management basic needs.
